Robert Hayes,

President and CEO

Robert Hayes (Bob) was named President and CEO of Community Healthcare Network in December 2014.

Mr. Hayes has deep experience in leading mission-driven organizations locally and nationally, both advocating for and delivering direct services to people in need. Mr. Hayes founded and led the National and New York Coalitions for the Homeless, winning the nation’s first right to shelter court ruling and battling to remedy many of the upstream causes of homelessness including the shortage of supportive housing for people with mental illness, substance abuse, HIV/AIDS and families in need of protective and preventive services. He has served as the president of the Medicare Rights Center, a non-profit organization supporting consumer interests in national and state debates over issues of healthcare and aging.

Most recently, Mr. Hayes oversaw consumer protection for more than two million Americans with Medicare as the senior vice president for health quality at Universal American Corp. Mr. Hayes has practiced law with firms in New York and Maine, including Sullivan & Cromwell and O’Melveny & Myers.

Mr. Hayes has an unwavering commitment to social justices and issues of equity. Mr. Hayes is a staunch advocate for healthcare, behavioral health care, housing, food and income support. He is a MacArthur Foundation fellow and has been awarded honorary degrees by 11 colleges and universities. Mr. Hayes is a graduate of Georgetown University and the New York University School of Law.

Matthew Weissman,

MD, MBA, FAAP, Chief Medical Officer

Dr. Weissman was named Chief Medical Officer of Community Healthcare Network in June of 2012. He currently oversees the quality of clinical care and operations of the centers, recruits providers, interfaces with community groups and hospital administrators, and provides a clinical perspective when collaborating with senior management on the operations of the organization.

Following the passing of CHN’s late past President and CEO, Catherine M. Abate, Dr. Weissman served as the organization’s Acting President and CEO from May 2014 to January 2015. Dr. Weissman joined CHN from the Ryan-NENA Community Health Center, where he served as the Medical Director. Prior to Ryan-NENA, he was an attending primary care physician at a community hospital in Yonkers, NY. Dr. Weissman received an AB in Economics from Harvard University magna cum laude and a Medical Doctorate and Masters in Business Administration in Health Management from Tufts University.

He completed his residency and chief residency at Mount Sinai Medical Center in New York and is Board Certified in both Internal Medicine and Pediatrics. Dr. Weissman holds academic appointments at Mount Sinai Medical Center and the Sophie Davis School of Biomedical Education and holds Courtesy Attending Physician appointments at Beth Israel Medical Center. He currently co-chairs the New York City Department of Health and Mental Hygiene’s Citywide Colon Cancer Control Coalition (C5). He is the co-chair of the Clinical Quality Committee and the Cardiovascular Project Committee of the Mount Sinai Preferred Provider System (DSRIP PPS).

He is on the Boards of Directors of the Community Health Care Association of New York State (CHCANYS) and AmidaCare.  His primary interests are the transition of pediatric to adult care, improving use of electronic medical records, increasing accessibility to care, and streamlining health care delivery. He lives in Manhattan with his wife and three children.

Elizabeth DuBois,

DNP, FNP-BC, AAHIVS, Deputy Chief Medical Officer

Elizabeth DuBois is currently the Deputy Chief Medical Officer at Community Healthcare Network. In this role, she oversees new initiatives, program development and operations of medical affairs. She also oversees the NP primary care and psychiatric NP fellowship.  In addition, she provides primary care services including adult, pediatric, gynecological, and HIV care.  She practiced at Georgetown University Hospital in Washington, D.C. as a registered nurse in several intensive care units. She completed her Masters of Science in Nursing at Columbia University and was a member of Sigma Theta Tau, the nursing honors society.

She served a clinical instructor at Columbia University during this time, teaching undergraduate nursing students, and functioned as the Nursing Coordinator for New York Presbyterian Hospital. She is currently a clinical instructor for the Nurse Practitioner programs at Yale University, New York University, and the University of Pennsylvania, and is also a clinical instructor for Doctorate of Nursing Practice students at Columbia University. She graduated with her Doctorate of Nursing Practice from Yale University in May 2017.

Elana R. Berman,

MPH, Chief of Staff

Elana R. Berman is the Chief of Staff at Community Healthcare Network. She holds a Master’s in Public Health from Columbia’s Mailman School of Public Health and a B.A. in Sociology from the State University of New York at Geneseo.

Elana was formerly the Associate Vice President of Member Advocacy at Universal American where she revamped the Appeals and Grievances department, managed acquisitions of new businesses from a compliance perspective. She also served as the Director of Client Programs at the Medicare Rights Center and as the Director of Volunteer services at Hudson River Community Healthcare.

She has 20 years of experience in the field of healthcare working for providers, community based organizations and healthcare payers.

Elana’s professional background includes working on issues of social and economic justice, non-profit management and strategic development. She also teaches Health Policy classes at Columbia University’s School of Social Work. Elana is bilingual in Spanish and served as a Peace Corps Volunteer in Costa Rica.

Dorothy E. Farley,

LCSW-R, Social Services and Care Coordination

Dorothy Farley is a licensed clinical social worker (LCSW-R) who received her master’s degree in social work from the Hunter College School of Social Work.

While she assumed the position of Vice President of Social Services in 1999, she began her career at CHN managing a teen program and would later become a center Social Worker. She has extensive experience on coordinated care delivery and social services.

In her time at CHN, Ms. Farley has overseen the successful transition of the organization’s COBRA Case Management Program from a fledging one team service in the Bronx to one of the largest COBRA CFP Programs in the state.

This award winning program enabled CHN to successfully transition as a New York State Health Home Lead in Brooklyn & Queens. Ms. Farley also oversaw the integration of the Article 31 Licensed Behavioral Health and Social Services program, which allows patients to receive their medical, behavioral and social services all in the same clinic. She currently sits On the Integration of Care Committee as a voting member of the HIV Health & Human Services Planning Council of New York City.

Kenneth W. Meyer

SPHR, Chief Human Resources Officer

Ken Meyer’s career in healthcare has spanned over 30 years with the last 25 years spent in the practice of Human Resource Management. Most recently prior to joining CHN, as an employee of Aptium Oncology, Inc., he managed the Human Resources Department for the NYU Clinical Cancer Center and the Stephen D. Hassenfeld Children’s Center for Cancer and Blood Disorders at the NYU Langone Medical Center. Ken is a graduate of the Herbert H. Lehman College and holds a Masters Degree in Industrial and Labor Relations from Cornell University/Bernard M. Baruch College. Since 2007, he has been certified as a Senior Professional in Human Resources through the HR Certification Institute.

Ken is also an active member of the local chapter of the Society for Human Resource Management (SHRM), the Human Resources Association of New York, where he serves as the Chair of the Managers’ Forum Special Interest Group. He is also the former president of the North Riverdale Baseball League, his community’s Little League and still volunteers his time whenever he is able. Luis Freddy Molano

MD, Vice President of Infectious Diseases and LGBTQ Programs

An advocate for quality healthcare for everyone throughout his career, Luis Freddy Molano, MD, worked as a family practitioner in South and Central America prior to joining Community Healthcare Network in 1989. Initially a health educator at our Community League Health Center and the Bronx Health Center, Dr. Molano went on to become Center Director at our Bronx Health Center for 12 years while simultaneously acting as Project Coordinator for the Adolescent Mobile Program and HIV Testing Program.

Dr. Molano then became Associate Executive Director for Women’s Health and the HIV Prevention Programs where he started and implemented one of the most successful HIV programs that integrated HIV counseling and testing with women’s health. As a result of his efforts he was named Assistant Vice President of HIV Programs and Services, a role he filled

for six years before being named Vice President of HIV Programs and Services in 2010. Dr. Molano is a respected voice on HIV issues, presenting at conferences including the US Conference on AIDS, Office of Population Affairs as well as State and City wide forums.

He has also written and published in medical journals like the American Journal of Preventive Medicine and The W-Path (Transgender Health Forum) among others. In addition, he is a Board Member of the Family Planning Advocates of New York State; on the Board of Trustees of the National Association of People with AIDS (NAPWA); sits on the New York City HIV Planning Council – Needs Assessment Committee and has served on the NYS Prevention Planning Group and the NYS AIDS Services Delivery Consortium and is a former member of the International AIDS Society. Alan Wengrofsky is the Chief Financial Officer and Vice President of Finance for Community Healthcare Network. He is a certified public accountant (CPA) licensed in the state of New York since 1993 and began his career with Loeb & Troper, LLC. where he specialized in not-for-profit and healthcare accounting.

Prior to coming to Community Healthcare Network, Mr. Wengrofsky was the CFO of a nursing home chain with annual revenue of $500 million dollars. He currently serves as a member of the finance committee of a private school and has previously served on the board of a not-for-profit agency.

David Gross

JD, General Counsel

David Gross, JD, is the General Counsel at Community Healthcare Network and oversees all legal operations of the organization. With over a decade of experience in healthcare policy, David was the former VP; Compliance Officer and Regulatory Counsel at Universal American Corporation, where he developed new compliance programs for each of UAM’s Medicaid lines of business as well as provided ongoing legal and regulatory support to Medicaid and ACO operations.

He also worked for the New York City Department of Health and Mental Hygiene (DOHMH), Division of Mental Hygiene on behavioral health issues and as a health care attorney with several national law firms. He received his JD from New York University Law School where he graduated cum laude and holds a Bachelor’s Degree in Political Science from Emory University.

Susan Yee

DrPH, MHA,  AVP of Community Health & Clinical Operations

Susan Yee joined Community Healthcare Network as the Associate Vice President of Community Health and Clinical Operations in November 2016.  Susan is a public health practitioner and healthcare administrator with over 15 years of experience in strategic planning, operations and process improvement, organizational development, program implementation and evaluation.

Prior to assuming her current role at CHN, Susan served as the Director of Quality Improvement and Strategy at New York University School of Medicine. She was also the Associate Director of Programs and Services at the Charles B Wang Community Health Center and the Deputy Director of Clinical Systems Improvement Unit at the New York City Department of Health and Mental Hygiene. Additionally, she has worked as an independent consultant for numerous non-profits in New York City.

Susan holds a Masters in Health Administration with a concentration in Health Policy from the University of Washington and Doctorate in Public Health from SUNY Downstate Medical Center. Her interest is focused on reducing health disparities, improving childhood and adolescent health and advancing the field of dissemination and implementation research.
