What is Care Coordination?

Care Coordination is a free program for adults with Medicaid. Our Care Managers work with eligible patients to coordinate medical and mental health visits and assist with other health concerns. Care Managers can also link people to community resources, such as food, housing, and legal services.

Access these services with Care Coordination:

  • Assistance with benefits/entitlements
  • Access to care
  • Advocacy
  • Escort to appointments
  • Linked to community-based services

To get Care Coordination Services you must:

  • Be over 21
  • Be an active, non-restricted Medicaid or Active Medicaid/Medicare member
  • Have 2 ongoing conditions OR HIV OR a serious and persistent mental health condition
  • Need care coordination services

What are the goals of the Care Manager?

  • The Care Manager talks with a person’s doctors and helps everyone talk to each other. This makes sure the patient gets the best care.
  • Patients get ongoing treatment and preventive care in their neighborhoods. This brings down hospital rates and ER visits while keep patients healthier.
