Welcome

Thank you for your interest in the first primary care nurse practitioner fellowship in New York State. We are so pleased to offer nurse practitioners an opportunity to train and develop advanced skills in the primary care setting. Community Healthcare Network has a long history of providing quality, comprehensive services to all New Yorkers and this care starts with an investment in training quality, competent providers.

 

The NP Fellowship is now offering two tracks: A primary care fellowship for family nurse practitioners and a psychiatric community health fellowship for psychiatric nurse practitioners.

 

With a national shortage in primary care providers, a newly insured population seeking care, an increase in complex comorbidities and a national focus on reducing emergency room admissions an opportunity to build upon skills learned in the academic setting is crucial. This full-time program provides a bridge into practice for NP’s and provides preparation for the challenging community health environment. The fellowship is designed to give the academically prepared practitioner an opportunity to spend more time under the guidance of a preceptor.

 

We are excited to expand our fellowship to include family NPs and psychiatric NPs, along with a growing cohort. As the fellowship evolves we may expand to other specialties including: adult, pediatric, and women’s health. Goals and Expectations

The fellowship program is designed to be rigorous and comprehensive. Fellows should be committed to the program which includes a full schedule of training, patient care, and didactics. Each track provides an array of opportunities that will advance skills and fellows should be prepared to take advantage and learn as much as possible by making this program a priority. Fellows learn over time and have constant access to expert providers who will help shape practice. A distinct aspect of this includes mentorship, networking and professional growth. We expect our fellows to have a strong interest working in community health centers on behalf of medically underserved communities. Fellows should have a passion for the primary care setting be committed to working a full week, plus occasional on-call and Saturday shifts for one year.

CHN’s NP Fellowship Program goals:

  • Increase the amount of primary care and psychiatric providers in the community healthcare setting
  • Provide recent nurse practitioners an opportunity to supplement skills learned in academia under the guidance of an expert provider
  • Increase workforce retention rate of providers in primary care, community health and medically underserved areas
  • Improve confidence, capability and job satisfaction of nurse practitioners

By the end of the year, Primary Care Fellows will be:

  • Proficient in primary care in an urban community health setting with a medically underserved population
  • Proficient in primary care in an urban, underserved communities
  • Capable of seeing 18-20 patients per day in a clinical setting
  • Able to treat a wide range of patients and conduct visits from prenatal to geriatric care
  • Able to problem solve with ease and determine appropriate referrals and referral sources

By the end of the year, Psychiatric Community Healthcare Fellows will be:

 

  • Proficient in psychiatric care in a urban community health setting with the medically underserved population
  • Experienced in integrating psychiatric care and primary care and working in a team-based approach
  • Capable of seeing 7 patients per day in a clinical setting
  • Able to diagnose and manage a variety of mental health disorders
  • Able to treat a wide range of patients of all ages
  • Demonstrate proficient knowledge of psychopharmacology

Eligibility Requirements:

Applicants must:

  • Be recently graduated family nurse practitioners or psychiatric nurse practitioners OR family nurse practitioner or psychiatric nurse practitioner students graduating before the start of the fellowship. Doctorate  students/candidates are encouraged to apply
  • Have a masters in nursing from an accredited institution
  • Have documents certifying they can work in the United States
  • Be Board Certified OR eligible to obtain a Board Certification

2016-2017 Primary Care Application Timeline

  • January 1, 2018 – Application opens
  • March 15, 2018 – Application closes
  • April, 2018 – Some will be offered interviews at CHN central offices
  • May, 2018 – Applicants are informed of final decision
  • September 10, 2018 – Fellowship begins

  • February 1, 2018 – Application opens online
  • April 15, 2018 – Application closes

  • 3 Professional letters of recommendation if not Board Certified, 2 if Board Certified. Dated, signed and addressed to Dr. Matthew Weissman, Chief Medical Officer. Letter should be from education program.
  • 1 letter from employment, and 1 of your choosing if not Board Certified.
  • If more than 5 years post-graduation, 1 letter can be from your current supervisor (in lieu of  education program), and 1 from your general  employment.
  • Letters can be sent independently or sent as  part of the application.

Frequently Asked Questions

When does the program start?

The Primary Care fellowship starts every September.  The Psychiatric Community Health fellowship starts every October.

How long is the program and what hours will I be expected to work?

Community Healthcare Network’s Nurse Practitioner Fellowship lasts for one year and starts in early September. Fellows will work a Monday through Friday schedule totaling 35 hours a week (7 hours per day with one hour lunch) and will take occasional Saturday shifts. Fellows, as employees, will also take on-call as scheduled.

What will the specialty rotations be like?

There will be rotations in both outpatient and inpatient settings. Inpatient rotations will take place in hospitals throughout New York City. Outpatient rotations will take place in various private practices and community based organizations located in the city. Fellows can expect to learn a wealth of information about specialty practice from these rotations.

Who will the preceptors be?

The preceptors will be a combination of experienced medical doctors, nurse practitioners and psychiatrists and psychiatric nurse practitioners employed by Community Healthcare Network.

What topics will be covered in the didactics sessions?

For the primary care track, the didactics each week will correspond with the specialty rotations as well as information needed when starting practice such as taking call, time management, reading diagnostic imaging reports, the art of referrals, interpreting consult reports, and communication skills. For the psychiatric care track, the didactics will include topics that range from psychiatric medication management to best practices with special populations. Significant emphasis will be made on working with the medically underserved population and those with co-morbidities.

I don’t live in New York City, can I still apply?

Of course! However, all fellows are required to have valid NYS RN and NYS APN licenses. In addition, we request that fellows live within the NYC metro area at the start of the program.

I’m not recently graduated, can I still apply?

Of course! We believe everyone can benefit from this program and leave it to your determination if the program is suited for you. We believe this program is especially fruitful for new grads.

I am not a Family Nurse Practitioner or a Psychiatric Nurse Practitioner, can I still apply?

Currently we are only accepting family and psychiatric nurse practitioners, as the fellowship evolves with more candidates we may expand to other specialties including: adult, pediatric, and women’s health.

What are the pay and benefits for this program?

What if I need time off?

The NP fellow can schedule paid time off in advance, but it must be approved by fellowship leadership. The fellowship includes 15 paid vacation days, 4 personal days, and 12 sick days.

Who are the leaders of the fellowship program?

  • Elizabeth DuBois, FNP-BC, is the Program Director she is also the
  • Katharine Frissora, DNP, PMHNP-BC is the clinical lead for the  psychiatric track. She is also the Senior Advanced Practice Nurse of  Psychiatry for CHN
  • Grace O’Shaughnessy, LMSW is the Program Manager. She is also the  Associate Director of Program Development for CHN.
  • Yuliana Perez is the Program Coordinator. Yuliana manages the  day-to-day operations of the fellowship.
