Family Program

Quality care for you and your family

CHN’s Veteran Health and Wellness Program offers short-term case management. All current and former service members and their families are welcome no matter your discharge status. This program can link you to social services, legal services, and healthcare. We accept TRICARE.

You and your family can receive the following services:

  • Primary health care
  • Dental care
  • Help with PTSD (Post-Traumatic Stress Syndrome)
  • Housing help
  • Legal help
  • Substance abuse help
  • Mental health care
  • Podiatry (foot care)
