Behavioral Health

CHN believes that true
wellness comes from a
healthy mind and body.

CHN offers behavioral health services at all of our centers for adults, children and families. Our trained team of Psychiatrists, Psychiatric Nurse Practitioners, Therapists and Social Workers work with our primary care teams to ensure that our patients are receiving care that will treat the mind as well as the body.

We offer these behavioral health services:

Social Work

CHN has trained social workers at all our health centers. They provide culturally sensitive and supportive counseling services. Social workers work closely with medical providers, behavioral health staff, care managers, and other clinical team members to help patients address medical and social service needs.

At a social workers visit, you can get help with:

  • New changes in your life
  • Believing in yourself
  • Homelessness
  • Stress or depression
  • Problems with drugs or drinking
  • Referrals to public benefits
  • Referrals to job training
  • Pregnancy counseling
  • Information about birth control
  • Problems with abuse
  • Managing your health care needs
  • Managing HIV and STIs
  • Immigration worries
  • Filling out a healthcare proxy
  • Worries about having enough to eat
  • Crisis intervention

Social workers work closely with medical providers, behavioral health staff, care managers and other clinical team members to help patients address medical and social service needs.

 

CHN social workers help patients figure out tough problems in life. Such as: family conflicts, relationship issues, parenting problems, and violence from loved ones. Social workers also give kind care during crises.

 

Social work services are free for anyone who gets care at any of our health centers. We help everyone, your immigration status does not matter. We have social workers who speak English, Spanish, French and can use translation services for other languages.

Behavioral Health

At a behavioral health visit you can get:

  • Help managing medicines
  • One-on-one therapy
  • Family therapy
  • Depression care
  • Help managing anxiety
  • Post Traumatic Stress Disorder (PTSD) therapy
  • Improve self esteem
  • Relationship problems
  • Stress reduction
  • Help communicating
  • Help with your moods

All of our health centers have many behavioral health services. CHN also offers depression care management at our Lower East Side Health Center and South Bronx Health Center. Our therapists speak English and Spanish. For other languages, we use a language line. We offer appointments during the day, evening, and Saturdays.

Addiction

CHN provides integrated addiction services. Here, you can be seen by your own primary care provider and a behavioral health therapist to treat addiction to pain pills or heroine. Patients can receive Medication Assisted Treatment (MAT) with Suboxone (buprenorphine/naloxone). Suboxone is a medication that can treat people who are addicted to opioids (pain pills or heroine).

Psychiatry

Psychiatrists and Psychiatric Nurse Practitioners are available at all CHN health centers. In collaboration with the comprehensive CHN care team, we set up a platform for integration of the mind and body, heart and soul. Psychiatric consultation, psychiatric evaluation, and individualized treatment plans are offered – from addressing lifestyle changes necessary to managing unhealthy levels of stress to medication management of depression, anxiety, trauma, mania, psychosis, or substance abuse.
