If you are LGBT, we understand your health care needs. CHN is a safe place for you to come and get quality healthcare and support. All of our centers will offer you a welcoming environment.

Transgender Family Program

CHN’s Transgender Family Program supports the lives of trans/gender non-conforming communities and improving HIV prevention and care. The Transgender Family Program creates safe spaces in low income communities. We offer services that address the individual goals of our clients.

Sexual and Behavioral Health (SBH) Program

The SBH Program is a free program at CHN’s Jamaica and Lower East Side health centers. SBH will provide you with support for your sexual health and mental health needs. The program will pay for some doctors visits and mental health care sessions.

How does the program help me?

  • Visits with a SBH Health Educator
  • HIV tests and link to care
  • Testing and care for syphilis, gonorrhea, and chlamydia
  • Hepatitis A,B, and C testing
  • Vaccines for Hepatitis A and B, HPV, meningococcal, and pneumococcal
  • 10 mental health counseling sessions with a therapist
  • Referrals to social services including: health insurance, legal help, work support, and housing
  • Education and help getting benefits for PrEP and PEP

To be a SBH Patient:

  • Must be uninsured (have no insurance)
  • Must get care at Jamaica or L.E.S. health centers
  • Must meet with SBH Health Educator your first visit
  • Must self-pay for other care or visits not listed here
  • If you are taking PrEP, you must sign up for PrEP-AP within 3 months
  • Must meet with SBH Health Educator every 6 months
